Your consent to receive telehealth services through Vytora Health.

Agreement to Telehealth Services

By using Vytora Health's services, you consent to receiving telehealth services, which include the use of electronic communications, information technology, and other interactive tools to provide healthcare services at a distance.

What Is Telehealth?

Telehealth involves the use of electronic communications to enable licensed healthcare providers to connect with patients using interactive audio, video, and data communications. This includes health assessments, consultations, diagnoses, treatment, and follow-up care delivered via electronic communication systems.

How Telehealth Services Are Provided

Vytora Health connects you with licensed, independent healthcare providers who are authorized to practice medicine in your state. The provider will review your medical questionnaire, health history, and any supporting information you provide. Based on this review, the provider will determine whether treatment is appropriate and, if so, issue a prescription if medically warranted.

All clinical decisions — including whether to prescribe, what to prescribe, and at what dosage — are made solely by the licensed healthcare provider. Vytora Health does not practice medicine.

Your Rights and Responsibilities

Privacy and Security

All health information transmitted during your telehealth visits is encrypted and handled in accordance with HIPAA and applicable state privacy laws. Please review our Privacy Policy and HIPAA Compliance Notice for full details.

Limitations of Telehealth

Telehealth is not a substitute for emergency care. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. Telehealth may not be appropriate for all conditions. Your provider will advise you if an in-person visit is necessary.

Availability

Vytora Health's telehealth services are available in 44 US states. We are currently unable to serve patients in California, Nevada, New Jersey, North Carolina, Texas, Massachusetts, Mississippi, New Hampshire, West Virginia, and Delaware.

Acknowledgment

By using Vytora Health's services, you acknowledge that you have read, understood, and agree to this Telehealth Consent. You confirm that you are 18 years of age or older and are located in a state where Vytora Health operates.
